Queen Margarethe is/was a heavy smoker who at one point was up to three (three!) packs a day. By all accounts, she still smokes. And she's 83. While not unknown, it's still unusual to have such a smoking history and make it to 83 without developing COPD or emphysema or lung cancer. I do wonder if there's other health problems in the background that aren't being revealed and was the main reason for the abdication?

In 2016 she gave an interview in which she plainly stated she would die on the throne because that was how it was always done. So she's clearly changed her mind in recent years.

My mom smoked a pack a day from 17 until 91. No COPD, emphysema or lung cancer.

That’s so not true. If that’s the case, why did the people around him let him stay at his female friend’s apartment? They had to know the optics of that even if nothing was happening.

Exactly. This wasn't innocent. He just didn't care. I bet he's done this many times before with Geneveva and evaded them for this long so he felt comfortable.
