How to address family with different last names on holiday card?

Oh man, what do you prefer when it's a blended family?

John Jones
Jane Smith, her maiden name
Larlo Miller from mom's previous marriage
Larlie Jones, their child together

I pick the adult I'm closer with (like if it's a coworker) and address the envelope to "Jane Smith and Family."

I thought this tradition died long ago. By calling yourself Mrs. Byron Franklin, you're removing yourself completely from the whole thing, making your identity ONLY as part of your husband.
