I think that picture is mostly because of the lighting and partly due to the fact she didn't wear much makeup. Plus the photographer took certain deliberate liberties with exposure or is just a terrible photographer. A pro photographer would never release that photo because it looks like a 12 yo took it was an autofocus point-and-shoot camera. Her whole demeanor is very respectful given where she was visiting. The outfit is modest, the makeup is modest, no weird facial expressions, and no red light outfit with 10 pounds of makeup like EK.

If you watch this video, there's nothing that looks like that washed out picture. She's south Asian, not an albino like that picture makes her look.

I actually think Usha did. JD and Usha are plain Janes. She was never going to land the hottest guy at Yale but she was at least a little better looking than JD when they first married and he was still chubby. Once he went on ozempic and got a beard to hide his lack of a chin they were much more evenly matched.

If Erika and JD were two random 30somethings at a bar there is no way she would look twice at him. He's only "attractive" to these women because he's Vice President and a not insignificant chance he's the future President.
