Be honest: Do you fake doctor's signature on forms?

I’m a dentist and I sign all my kids school dental forms (kids are not my patients). I always have a doctor sign the other forms tho.

So you drop off a small stack of forms, with several business days’ worth of lead time. Then you pay the fee. This is not hard, people. It requires a little planning, organization, money and patience.

If you can’t afford the doctor fees, you can’t afford the camp. If you aren’t responsible and accountable enough to abide by the camps’ policies, you have no business sending your kid to the camp. It is that simple.

Our pediatrician will fill them out for free at the annual well visit, if you bring the physical forms with you and give them to the nurse (generally we have to pick them up later, though, especially if it's more than one).

We're fortunate that my DD has a May birthday, so by then we generally know which camps she'll be attending that summer. But in the case of one camp that required the forms to be submitted before that year's visit, I just let them know I'd be submitting it after her checkup, and they were fine with it.

It’s not per camp. We have our Dr fill out a blank Virginia School Health form at the annual well visit. It’s free when we bring it with us, and $35 if we need to request it special after the fact.

We fill out everything except the name of the school / camp at the top of the form. We scan it when we get home and then print it out and fill in the camp name as needed.

Also my kids have attended probably a dozen different camps over the years. The form doesn’t have to be recent - just less than 12 months from the start of camp. I get my Nov BD kid’s form for the year in November. I get my July BD kid’s form in July. I submit the prior year form at sign-up and make a note that I will send the new current year form in July per his BD.

We had to pay an extra fee once because my husband didn’t know to bring the form with him to the well visit. We scan the form and put it in Google Docs where we can both access it.

I wouldn't impersonate a specific individual, but I'd sign the sheet myself. Really I wouldn't go to a nonsense camp like that, by my SO would overrule me.

What kind of unlicensed, uninsured camps do your children go to that don’t require forms? What state do you live in?

I haven't but it's crossed my mind. My child has a food allergy and for whatever reason, neither school nor camps seem to give me the form at the same time as the normal medical form.

My practice needs several weeks of lead time during the busy times of year (camp and school starts). I have to admit I "signed" a form one time when I forgot to give the doctor the last page of the form. The pediatrician said it would be another $35 and two weeks to process the single page. No regrets.

Ever camp we use has a different form. Are you saying all your camps use 1 form?

Is DC and Northern VA is usually the same government form for camps as the public schools use.

I’ve often said there needs to be a camp health central database. The amount of forms is such a waste of drs and parents time!
But I do it anyways.
