No but I'd like to. The forms have to go through the "medical records office" and need tons of lead time. And it's not that we actually need another physical, it's that every camp requires a separate signed medication administration form. Day care even requires a doctor's signature to administer sunscreen, a new one every spring and fall along with a new tube of sunscreen, which is just a ridiculous hoop.
So you drop off a small stack of forms, with several business days’ worth of lead time. Then you pay the fee. This is not hard, people. It requires a little planning, organization, money and patience. If you can’t afford the doctor fees, you can’t afford the camp. If you aren’t responsible and accountable enough to abide by the camps’ policies, you have no business sending your kid to the camp. It is that simple.
My practice needs several weeks of lead time during the busy times of year (camp and school starts). I have to admit I "signed" a form one time when I forgot to give the doctor the last page of the form. The pediatrician said it would be another $35 and two weeks to process the single page. No regrets.
