Per camp.[/quote] It’s not per camp. We have our Dr fill out a blank Virginia School Health form at the annual well visit. It’s free when we bring it with us, and $35 if we need to request it special after the fact. We fill out everything except the name of the school / camp at the top of the form. We scan it when we get home and then print it out and fill in the camp name as needed. [/quote] Ever camp we use has a different form. Are you saying all your camps use 1 form?[/quote]
