Per camp.[/quote] Our pediatrician will fill them out for free at the annual well visit, if you bring the physical forms with you and give them to the nurse (generally we have to pick them up later, though, especially if it's more than one). We're fortunate that my DD has a May birthday, so by then we generally know which camps she'll be attending that summer. But in the case of one camp that required the forms to be submitted before that year's visit, I just let them know I'd be submitting it after her checkup, and they were fine with it. [/quote]
