The pattern in my preppy-ish family is for the first name to be something classic and old-fashioned (Sarah, Caroline, Anne, etc), and the middle name is a family surname.

Some are called by the first name or a variation of it, some are called by the middle name. And some are called by a somewhat wacky nickname that has no relation to either the first name or the middle name.
