It would help if you gave us a little bit of a description of the guy and/or his interests. Career? Pastimes he enjoys? Sports he plays? What does he like to do to relax? etc? I can say that I can think of 5 guys around his age right now who would have 5 completely different lists of things they'd enjoy as gifts.

It depends on what he likes, however the concept here is for a man who has known you for 20 years - you need to give something timeless.

If he's into guns, perhaps a nice engraved 1911 or lever action rifle.

if he's into art, perhaps an original work in he style/genre that he would like that he could keep forever.

if he's into experiences, perhaps gift him one of those national geographic treks to a cool place.

is your husband not weirded out by this? I only ask because my best friend is a male, we met the first day of college, and even though we were never romantically involved, my husband is weirded out by our relationship which he won't admit. If I was putting real thought into a gift I was getting for my friend my DH would be jealous and pissed.
