Bangladesh and then India. Imodium is actually awful if you get giardia or something similar. Cipro is your friend.

Both times in the US:

- Wicked food poisoning from salmon at Vidalia in DC nearly 20 years ago.

- Was hospitalized for several days after picking up something at Cosmos Club in DC 12 years ago. Literally had blood pouring out of me.

So, be careful when eating in DC ;0)

Weirdly, Israel. Nowhere I've been in Asia, S. America, Africa, Europe, etc. has made me that sick. I'm very lucky that I've never vomited from food poisoning in my life...but man, the shits from whatever I ate in Israel were BAD. I probably let my guard down too much because it's so developed.

Paris. Night before we left my best friend and I had oysters at a fairly high end place and got food poisoning.

Mexico. I think from a non-touristy restaurant on the way back from some ruins. We ate the fruit, presumably washed in local water. So be careful with fresh fruits and veggies too.

India. Multiple days crying on the bathroom floor. Stealing towels from the hotel so I could diaper myself on the train back to a city where I could get to a doctor.

Bali - but I let my guard down and ate at a lot of local spots. I've visited tons of other places throughout Africa and Asia without ever getting sick but was more careful about food in those places. Like in India, I mainly stuck to food in really nice hotels or other high end, well-regarded restaurants. So not as interesting but i didn't get sick. But then I've also eaten at roadside stands and super dive-y places during a month long backpacking trip thru the Middle East and never got sick. I mainly just ate felafel, hummus and pita literally every single meal (was on threadbare budget). Maybe that kind of food is less prone to hosting bacteria.
