Her breasts are clearly full of milk in this picture -- engorged shape, prominent veins and everything. Is this picture posed? Yes. Probably one was BFing and one had stopped. Is she really feeding both of them? I'd guess yes. Plenty of people tandem nurse older and younger children, especially in attachment parenting/other hippie-ish communities... but also in some religious ones. Lots of people also feed their babies previously frozen breast milk that was, shock horror, pumped at an earlier age and the kid survives.

Look, Hilaria is awful. But this accusation is nuts.
