4 miscarriages in a row. It’s my egg quality, right?

IVF was a total failure - DOR and 3 cycles where I never made blasts. Then in the 2 years since stopping IVF I’ve had 4 miscarriages - one blighted ovum and 3 early losses between 5.5-6.5 weeks. I’m only 36, but its pretty clear I’m just out of good eggs, right? RPL testing has all come back clear. The only thing left is immunology and I’m really hesitant to go down that rabbit hole when it’s probably going to be more time and money for more bad embryos.

Was thyroid testing part of your RPL panel? Just a shot in the dark here.

I'm sorry, OP. That is all very hard. I know from personal experience. I had a very similar track with multiple rounds of IVF where I produced one or two eggs on the highest drug protocols and none every became blasts. I did 8 rounds of IVF between 31-36. It was, indeed, a problem with the quality of my eggs. I never did immunology testing either since the poor response to the drugs is much more indicative of DOR (or POF at that age) than it is an immunological problem.

I moved to donor eggs at 37 and had a baby on the first try with DE IVF at 37. I then had another at 40 and another at 43, each time on the first try. If you are willing to consider donor eggs, it has such a higher success rate, especially for people who can get pregnant easily like you, where there isn't an implantation problem or a uterine problem. I can't say I wish I had tried it sooner because then I wouldn't have my exact three amazing kids and I wouldn't trade them for anything, but I do wish I could have all those years of heartbreak back. I hope that you can find an outcome that gives you peace.

36 is still pretty young for fertility issues. You should have a recurrent miscarriage panel done. Absolutely check your thyroid. You want tsh below 2.5, ideally closer to 1.0. If it's over 2.5, get it treated, even if you have to doctor-shop to get it done.

Read up on egg quality. Supplements do help. Vitamin D is another thing that impacts ovarian function. It takes about three months to improve egg quality. I'd do some reading here - https://itstartswiththeegg.com/

Sorry, I missed that you did the RPL panel already. I would still look at tsh, as not every lab follows those standards. And vitamin D. And look at supplements to improve egg quality. I had three ivf's fail, was a poor responder, had a blighted ovum, had one ovary removed at 36, and managed to have two kids, one at 41 and the second at 44. Oh, and I have hashimoto's so poor thyroid functioning. I tried just about everything, and I really believe supplements, vitamins, proper treatment of my thyroid, acupuncture all came together to help me finally have a successful pregnancy.

OP here. Thyroid was 1.3 during the RPL panel. I was on all of the supplements for years, never seemed to help during IVF or after. Time between positive tests is 9-15 months, so subfertile, but yeah implantation happens with some regularity. I know donor is the next step, but every positive test brings some hope and it’s hard to let go of the idea that it can work out eventually.

I’m looking at my RPL panel results again. Anything besides TSH and vitamin D I should be double checking?

Even if your thyroid was still in the normal range. If you haven't it can't hurt to be looked at by an endocrinologist. Regular doctors don't always look at thyroid testing or don't know how to interpret it.

Have you considered sperm quality? It's not always the egg, even though that's what many doctors just think of.

Agree with pp. Uterine lining is more likely at your age. Endometriosis?

For me it was uterine issues, likely caused by a D&C. Hugs to you.

OP here. Sperm and DNA fragmentation are normal. Had endometritis which was treated and normal, no other uterine issues found. Thyroid was good, not just normal, and no symptoms of thyroid issues. I appreciate the suggestions, all, good to think everything through to make sure we haven’t missed anything.

I don’t think so. It’s pretty unusual to get pregnant three Tunesi naturally with DOR.
