So Tuesday at 7 weeks along, I found out I’m having another miscarriage. My first ultrasound at 6 weeks, we saw gestational sac, yolk sac and fetal pole measuring exactly 6 weeks (no HB yet). Went back yesterday and the tech said she just saw sac and yolk sac and no fetal pole and my gestational sac only grew by 1 day in the last 7 days.
This pregnancy came after 2 chemicals in a row (end of November & beginning of January). So technically this was 3 losses in a row. I also had a miscarriage last March at almost 9 weeks along (at 8 weeks 6 days, baby only measured 7 weeks 3 days with no heart beat). I am 31 with pcos and hashimotos that is well managed by my endocrinologist. I am on NAC, ubiquinol, myo & d chiro inositol, levothyroxine, metformin, baby aspirin, vitamin d, omega 3 and prenatals (Naturelo brand). I continued all of these supplements while pregnant (per approval by my RE). I’m so beyond frustrated at this point. After my 2nd chemical in a row in January, my RE offered me the RPL panel which I happily did. He told me everything came back completely normal. My TSH is also normal, prolactin normal, A1C normal, etc. Everything that could be checked was checked and came back fine. My husband and I also did a full genetic screening back in the Fall and all was clear. I’ve had an HSG and saline ultrasound done in November and both came back clear. Endometrial biopsy came back clear. So I just don’t know what to do at this point. I can clearly get pregnant ( with help of letrozole), but they aren’t sticking and I’m almost to my breaking point. Does anyone have any ideas, tips, stories, etc.? I just don’t know what can possibly be the issue. |

I’m sorry this is happening to you and I wish I had some advice. All I can tell you is my own experience, as someone with PCOS and sub clinical (but treated) hypothyroidism and everything else completely normal, including husband’s numbers. This spanned my early to now later 30s. Here’s what happened to me:

For me, they found a fibroid that we’re certain caused the last miscarriage, but otherwise, just...bad odds. It sounds like you’re doing what you need to do. One final step could be IVF with testing of all the embryos, to see if it’s something embryo-specific or uterine specific? |

IVF with PGS would be an option, if chromosomal issues could be causing the miscarriages?
One of mine was a missed miscarriage at 6/7 weeks that required a d&c. Chromosomal testing showed a trisomy. |

| I'm so sorry you are going through this. It's hard (and sometimes impossible) to pinpoint the problem. If you haven't yet miscarried, I'd recommend testing the baby. I've done this twice after miscarrying at home, so you can do it even if you don't opt for a D&C. |

I'm really sorry, BTDT and I know four in a row is terrible and messes with your head so badly.
I agree IVF with PGS/PGT-A. You could just be unlucky and have lower egg quality than you should for your age (I do too). You could also consider sperm DNA fragmentation testing if you go down the IVF route. It's a bit fringe still, but the theory is highly damaged sperm has too much DNA damage for the egg to repair (especially if you have lower than average quality eggs) and that can lead to implantation failures, losses, and genetic issues with ongoing pregnancies. It'll probably be a waste of $500, but when you get to that point, sometimes peace of mind in ruling out every possibility is worth it. Hugs and good luck. |

Op here- that’s honestly what I’m thinking. Is there something I should be asking my RE or endo to look more into? My tsh was 0.89 a week ago so that’s been fine. He also did a full thyroid panel a month ago. |
I’m so sorry, OP. I also have thyroid disease. I have not a ton of research on this but if you search old posts on this forum you might find info on autoimmune and infertility. Some have had cycles where they are taking steroids to surprise the immune system at time of transfer, since autoimmune diseases put your immune system into overdrive and can attack the embryo. Again haven’t done much research but maybe worth looking into? Wishing you the best of luck. |
It's not your hashimoto or your thyroid, it's that if you have 1 auto-immune condition you're more likely to have another Get a specialist who sees lots of auto-immune infertility people |
Actually hashimotos alone has proven to correlate with a high miscarriage rate. But same action item. See an RE fluent in autoimmune issues. Also OP I’d consider PGT-A too because it could be chromosomal issues, at least for some of your losses. Also get checked for endometritis if you haven’t already. |
| I’m sorry OP. I have hashimotos and had three miscarriages before a live birth. We finally tried a low dose of prednisone during my first trimester and it worked x 3 kids later. If you google prednisone and recurrent miscarriage there are some studies you might discuss with your doctor. Again I’m very sorry. That was a dark year. It felt like the sun never came out. |
Op here- thank you so much. I actually had a friend recommend the “immune protocol” of prednisone, Claritin & Pepcid. She also has hashimotos and had 3 losses between 6-8 weeks and conceived the first cycle on that protocol and had a healthy baby. I’m definitely going to bring it up to my RE next week. |
| I had terrible PCOS, like 3 periods/year. I got it under control with vitex and progesterone cream. Between dc1 and dc2 I had a MC at 7 weeks and didn't use the progesterone. When I got pregnant the 3rd time, at 40, I used the progesterone cream the whole first trimester. |
