As soon as vaccine rates fall below a certain rate, lots of people are going to get sick again, not just the Amish. Herd immunity can only protect the Anti-Vaxx free loaders up to a point.

As someone who works in pediatrics this is so depressing.

Educated people with resources will get advice from their physicians and get their kids vaccinated as recommended . I can't imagine why insurance companies would not cover vaccines because it's a lot less expensive for them to cover the measles vaccine then the measles hospitalization.

Tough to say. Historically, wealthy elites have been likelier than population average to come up with a bespoke vaccination policy.
