We are in a similar position, and they told us to turn in the forms now (without all of the three-year old requirements done) and turn in another updated form as soon as third birthday comes.

It was central office that started calling in late June/early July (office of early childhood something-rather). I would schedule the exam for early August because they probably will not let you start without it.

^^to be clear: you need at least one dental exam before August. If you have had one, have your dentist fill out the form based on that. Otherwise, schedule your dental cleaning for the summer before school starts. For the health form, your dr can fill it out based on your last physical, and then you update the form based after the 3 year exam.

The Dental form isn't that hard or complicated. In order to attend a DCPS (or charter) school you must have a form filled out by a Dr. and that form must be dated less than one year before it is turned in. That form is then good for the entire school year. Your kid needn't be 3 to have the exam. Just take your kid to a Dr now and submit the darn form.
