If you're saying "per", as in "per the court documents, he is not to be around children" or "per the email from John, Mike will be there" you don't need the word "as"! I don't know if it's a new thing or I just never noticed it, but man, the number of people who write out things like "as per the itinerary, we will get there at 5" or "as per her schedule, she will not be there" is crazy.
