Highly unlikely as she would have to be pretty far along for any surgery on a fetus. It could have been something like ectopic pregnancy with complications, but unlikely that the recovery would be so long.

I tend to think it’s health related, not a marital issue because a divorce would eventually come to light and they’d be caught lying. Also, if they were in the middle of a marital issue, I think William would be going overboard with public appearances to try to improve/maximize his image for the inevitable divorce backlash. Rather than posting a badly photoshopped photo or Kate and the kids, he would have posted a happy family picture that included him:

To the extend there are marital issues, it’s in both their best interests to keep status quo, living increasingly separate lives rather than to separate.

I went from thinking the coma rumor was crazy to now wondering…

I thought she just wanted privacy but now that they released a terribly doctored photo I also am now inclined to believe conspiracy theories. I hope she's ok.
