I tend to think it’s health related, not a marital issue because a divorce would eventually come to light and they’d be caught lying. Also, if they were in the middle of a marital issue, I think William would be going overboard with public appearances to try to improve/maximize his image for the inevitable divorce backlash. Rather than posting a badly photoshopped photo or Kate and the kids, he would have posted a happy family picture that included him:

My sister was put into a coma when she ended up in sepsis shock as a result of a tiny little kidney stone. It can happen with anything. She was in it and intubated for about 48 hours. She recovered well enough to be released three days later, but her recovery coming back to full scale energy and normalcy took about two months.
