I think he and his wife were likely separated before he went in to rehab. From her statement it sounds like maybe she thought they would get back together. I wouldn't be surprised if John was having an affair prior to rehab though.

She's literally 41 years old and was likely on birth control for 20+ years. It's highly, highly unlikely she got pregnant without IVF.

It usually means they're no longer sexually attracted to the wife (and possibly already having an affair).
