If you never let them face out it tends to not be a problem. There are good reasons to not have them face out -- the body positioning being one but over-stimulation can also be a concern, as they have nowhere to tuck their face away.

If a caregiver really wants baby to see out I'd recommend either a hip carry with a ring sling or pouch (shorter periods of time, really, being that it is a one-shouldered carrier) or a high back carry with a mei tai or woven wrap. If they're up high on your back they can see over your shoulder but also have a place to rest their head if they're feeling over-stimulated.

The Pikkolo is a good carrier. Nice option if you are insistent on having baby face out. (Sorry, that sounded kind of snotty but it wasn't how I meant it).

Thanks - I'm not insistent but I like the newborn feature as well and the facing out is a plus... because DS hated facing towards me in the Ergo.
