+1 Behavioral therapy plus medication was very helpful for us. 4 and 1/2 may be too yoing to medicate, but you can start behavioral intervention anytime and add medication later if indicated.

If somebody whose thyroid wasn't bust took the amount of thyroxine I am on they would spin in circles, if I stopped taking it I wouldn't be getting out of bed in about three days time and probably die eventually. I will never understand why people use "This totally healthy 20 year old took x and had these side effects" as a reason not to medicate a child with a medical condition.
