Diphtheria has three primary characteristics:

1. a "bull neck" with edema and lymph node swelling obscuring the neck landmarks
2. a "pseudomembrane" composed of dead skin and friable tissue obscuring the airway
3. Heart complications and other issues that show up as the child is improving

These children used to choke to death. One of my mentors tried to intubate a child in the far north, and he just pushed the pseudomembrane down into the lungs as she bled out.

THIS is the disease that finally lead to acceptance of cricothyroidotomy, or punching a hole in the neck to establish an airway. Doctors historically were unwilling to do it, but they started to save some lives with diphtheria this way.

That bullneck? It obscures the landmarks, and you have trouble finding where to put the tube.

So what do you do? Treat the Corynebacterium bacteria with antibiotics, but it releases more diphtheria toxin as it dies, so there is a delicate balance. Get an airway established with a hold in the neck if they are even moderately ill, because it gets worse fast. Keep them alive if you can, and then deal with the subsequent mycocarditis and neuritis if they occur.

tetanus isn't that great either


Tetanus is horrible, but there is not much you can do. It's all supportive care. That would be hard on hospitals and the infrastructure, but -- to be frank -- it doesn't require special training to watch someone die when it is inevitable, as there is no treatment.

Diphtheria (Corynebacterium diphtheriae), an acute bacterial infection spread by personal contact, was the most feared of all childhood diseases. ... One of every ten children infected died from this disease....

In the 1880s Dr. Joseph O’Dwyer, a Cleveland native, developed a method of intubating patients (inserting a tube to keep the airway open) [cricothyroidotomy] to survive the life-threatening phase of diphtheria. Although neither foolproof nor simple to use, O’Dwyer’s intubation instruments comprised a life-saving last resort. ... Diphtheria became a seldom-seen threat to children, but only so long as they had been vaccinated.

Before Dr. O’Dwyer perfected his intubation techniques, tracheotomy presented the only viable treatment for diphtheria. This procedure involved cutting open the throat without anesthetic and inserting a tube directly into the trachea. Through this tube, an attendant could maintain consistent airflow by pushing air into the lungs....

During the early 19th century tracheotomy remained a last resort due to the lack of anesthesia, high risk of infection, and low success rate of the procedure.

Diphtheria vaccination first appeared in the 1890s, but only became widely used in the 1920s. During this interval medical scientists labored to create a safe and effective vaccine. ... In 1924, Gaston Ramon developed the toxoid, a neutralized form of the toxin that would still impart permanent immunity. The toxoid-antitoxin mixtures eventually developed into the TDAP vaccine that is still in use today.

Cleveland did not escape the diphtheria outbreaks of the 19th century unscathed. In 1875, the 243-person death toll from diphtheria comprised 8.2% of all reported deaths. As was typical of the disease, children comprised most of the mortalities.

My grandfather’s brother died of tetanus at age 10. All of his siblings lived into their nineties. His mother never got over it and lamented his death till her last day. It’s mind boggling that someone would not avail of preventative measures.

By the way, the modern diphtheria vaccine was invented in the 1920s.

Prior to that, physicians were injecting children with antibodies filtered from animals infected with diphtheria. And after that, in the early 1910s, there was an antibody-antigen complex that served as a vaccine.

This is disinformation. I’m not trying to minimize tetanus, but it erodes trust when the believers of science propagate provable falsehoods.

This is such a lazy canard. A central tenet of our healthcare system is that patients should be given informed consent before a treatment. That usually means a conversation with a doctor. People like you consider that unremarkable yet guffaw at the notion that a motivated layman could ever research their way to a better understanding of a medical question.
