She can't be at fault, because there were two pilots, and it was the other's job to watch closely and take over if there was any mistake or danger. Also, the voice on the audio clearly telling the ATC that he saw the plane was obviously male. |

Were they drunk? Distracted? Not participatory in the expedition? All 3 had an obligation to pay attention. If this was human error, then all 3 failed. |
