So I’m annoyed. The Dr upped my menopur to 225 and 225 of Gonal F. I asked the nurse why when we are trying to use less menopur. The nurse said my estrogen was only at 86 today. The LH in menopur slows down the production so the eggs have time to mature. But I still don’t get it.

When others have done lower doses of Menopur what was your protocol?

150 Follistim, 75 Menopur. Still had more than 10 follicles at age 42. Once I had 225 Follistim, 75 Menopur and got even more eggs.

If they are not willing to change up your protocol after so many attempts at the same old thing, I would move on. You've tried it their way 4 times now. I don't see why they won't work with on this.

Is using a higher dose of Menopur (150 - 2 vials or 225 - 3 vials) always considered a not ideal thing? Or is it only a concern if you haven’t responded well on those higher doses? Just curious because when I see comments about avoiding higher doses I’m not sure if that’s meant to be across the board in all circumstances or for specific situations?

I agree. I told the Tech this morning that I don’t want some random nurse telling me I need to up the menopur because I’m starting Ganirelix. She was shocked but I don’t care. She said there are 14 measurable but the lead one is 11. She thinks the doctor will up the menopur. They want to see my estrogen higher. It was only 86 on day 4. But a few cycles ago the estrogen in day 4 was 85 and a few times it was 230. So I don’t really care what it is on that day right? That’s not changing anything for me.

I think there is a standard amount REs prescribe but I’ve read about (and people have commented) that if a patient is not getting any blasts lowering the amount of Menopur could improve quality.

After two failed cycles with no blasts, I asked my doctor to reduce the amount of Menopur we were using. I didn’t get any support on that request either, OP. I felt like my doctor was dismissing my theory because he thought I read about it on snakeoil salesman type websites.

PP from upthread whose wife moved to Cornell - your doctor should check out this study. My wife participated in it and was chosen for the mini IVF arm of the study. It resulted in our DS, now 5. Almost every woman in the trial who did mini had success.

Thanks. I’ll take a look.

I saw the doctor when leaving my appt today and mentioned my wanting the Menopur to be lower. She agreed but wanted to see the estrogen today. So now I’m at 225 menopur and 375 follistim. I don’t even want to be that high in the menopur. Anyone do this combo or did you do lower menopur? I’m REALLY tempted to only do 150. Thoughts?

Ok OP here. Most eggs ever.

21 follicles
20 mature eggs
Split into 2 groups:
16 with conventional IVF (not ICSI) with husbands sperm and 4 with sperm donor egg using ICSI.
We find out fertilization rate in the morning.

Definitely most mature eggs.

Hey OP, did u ever get pregnant and lost it in any of these 11 cycles or no embryos ever went past day 3. Sorry trying to understand the situation..
I am kinda like you , but got blast on day 6; nothing to freeze; 3 pregnancy loses so far.. so I know at least 3 times I had blasts that implanted.. furthest I went is to 8 weeks of pregnancy..

Only natural pregnancy 3.5 years ago. Got one blast on the 2nd retrieval and it was abnormal.

Nothing really makes it past day 3, all start to degenerate. We have had a few make it to cavitating morula or early blast on day 5/6 and then just stop.

Got the fertilization report:
20 eggs
7/16 of husband's sperm fertilized on their own
2/4 donor sperm (icsi) fertilized

Thanks for the update! So what dose of Menopur did you end up on? Did you stick with 225 in the end?

Stuck w the 225 in the end. If we do one more cycle, I will stay at 150.

Report on the fertilization and then day 2 division

Donor 4 eggs, 2 fertilized.
1-three cell with 30% frag
2- no change from yesterday

Husband's sperm- 16 eggs, 7 fertilized.
1- 2 cells, 10% frag
2- 3 cells, 30% frag
3- 2 cells, 20% frag
4- 5 cells, 20% frag
5- 3 cells, 20% frag
6- No change from yesterday
7- 3 cells, 20% frag

So total of 7 days 2 embryos anywhere from 2 cells-5 cells with 10-30% fragmentation.

This sort of tells me the Dr. is wrong and that the issue is not the sperm, right?

Anyone have similar results and get pregnant?

No update as they don't want to disrupt and slow down the embryo growth.

I spoke with the doctor and her take away was that right after fertilization....before the 2 whatevers fuse, the egg itself has fragmentation that is not consistent with my age. WTF. Now what?

I will get the report on the embryos tomorrow. I suspect that they have all stopped growing, as usual.

Has anyone greatly improved egg quality?

The doctor isn't calling this DOR--my numbers are great and I produce eggs like crazy.

I am going to try another cycle with very little menopur....I want to see if that makes a difference.

But, in the meanwhile, what are tricks of getting better egg quality? Already doing COQ10 and therologix vitamins.

Are you taking anything else that might be contributing to egg frag? Your embryo frag rate of 10-20% aren't bad. 30 percent is high but you don't have that many with that stat in the cohort fertilized with your husband's sperm.
