Amazon refunding an item different from what I returned

Three times in the past 6 months, the following things have happened:

1. I initiate a return for Item A and return Item A to UPS
2. Amazon immediately refunds me for Item A that is "in transit"
3. Amazon sends me an email saying they received Item B and refunds me the amount for Item B
4. Amazon sends me an email saying "You said you were returning Item A, but we received Item B. We are going to charge you for Item A now"
5. I contact customer service, they said they took care of it but then a month later I get charged again for Item A
6. I contact customer service again, they said they are going to refund the charge, but they don't.

It's always two items from the same order and Item A is ALWAYS more expensive than Item B.

I double check and every time I DID return Item A, and I STILL HAVE Item B. I never even tried to return Item B. The order history even says that I initiated the return for Item A. I didn't do anything wrong!

Example: I ordered after market all weather floor mats and a trunk mat for my car. I returned the floor mats. Amazon refunded me the floor mats. Then two days later I get an email saying "We have received the trunk mat and refunded you that amount". That trunk mat is IN MY CAR. I didn't return it. A couple months later "We have not received the floor mats. Please send them or we will charge you for them". A couple months after that "we never got those floor mats, we are charging you." I have now contacted customer service five times and the last time they said they would refund me, but they haven't yet.

Has this happened to anyone else? It's happened three times with three sets of completely different items (swimsuits, car accessories and this most recent time it was a water bottle instead of leggings)!! It has never happened to my husband before or anyone else I've mentioned it to, so is it just me? What is going on???

It happened to me once but the mistake was in my favor. That sucks.

Amazon sucks. They pull so much crap like this and closing accounts with gift cards saved to them. And all the counterfeit good.
I avoid using Amazon as much as I can.

This happened to me at Zappos which is now owned by Amazon. It used to be great and now it is a wreck. Avoid as much as possible.

OP, as PPs said you just need to avoid. You seem to be buying an returning a lot of stuff.

Use your phone amazon app and try to get customer service. Talk to someone.

Take a picture - which I know is a pain. But having visual evidence helps.

Have you tried returning to a Whole Foods Store - I know that is not easy for everyone.

I've returned so many things over the years, usually from orders with multiple items, and this has never happened to me.

I always return at Whole Foods, and use the QR code (either with the handy return machine, which works so well, or with an actual human). Could you change your mode of return next time and see if it makes a difference?

Clearly, there's something wrong with the way UPS manages the Amazon return items. Somewhere, the scanning/identification of the returned item gets confused with the item you kept.
