Why “Kate Middleton”?
She’s not Princess Kate, She’s Her Royal Highness The Princess of Wales.
Only Princesses of the Blood Royal are entitled to use HRH Princess Givenname. Since 1917 daughters and male line descent granddaughters are styled and titles this way. Before 1917 Great granddaughters in the male line were Princesses with the staple of Highness. Now they are titles and styled as the daughter of a Duke.
Women who marry in are either fortunate to marry a royal duke and then they are titled and styled HRH the Duchess of XYZ. A few are stuck like that old witch HRH Princess Michael, who we all know years to be called Princess Marie-Christine (chortle), with their husband’s name (as in the US many women were and some are Mrs.John Jomes).
QEII made a special exception for her aunt (by marriage) in recognition for her service to the monarchy. When Prince Henry, The Duke of Gloucester died, Alice did NOT want to be HRH The Dowager Duchess of Gloucester. The Queen allowed her to be styled as HRH Princess Alice, The Duchess of Gloucester.
In any case, I think Kate Middleton was in the picture for such a long time that the press were in the habit of using that, and the public knew her well that way, and jt just sort of continued. When Meghan came along, it was natural to follow the same habit, and she had minor fame under that name already.
