It’s fine if you want to take some space OP. Ultimately you accepted as part of the relationship that you would remain unknown to his wife and family, so you need to understand what that means. But you don’t have to like it.

It sounds like he has you strung out on the “special” place you have in his life. Honey, please understand that you are special without him and you don’t need to keep filling this role. Take whatever good he gave you, wish him love and move on.

It's not hard to google names of surgeons or pictures., OP. The bottom line real or not this man does not want you in his life in any real way. At the best, you are his dirty little secret. You are of zero value to him. Stop communicating with him and get therapy.
