Any over 35 moms elect for CVS/Amnio after negative NIPT results?

Anonymous
I got my NIPT results last week and everything is negative, thankfully. But I know that is just a screening and not a diagnostic test. I am considering doing a CVS/amnio for more certain results, but of course I do not want to risk m/c (although I understand that risk is extremely low with an experienced MFM). I'm going to see what my MFM says next week at my NT scan (I will be exactly 12 weeks then). Just curious how other AMA moms made this decision. I will be 37 at EDD.

Skipped right to the CVS. It was fine.

Thanks. Just curious, why CVS and not amnio?

What are you concerned about, exactly? I’m AMA and this has never crossed my mind.

Well, I'm not actually concerned about something specific, other than the fact that because this is only a screening, it can't tell me with certainty that the baby doesn't suffer from various abnormalities that NIPT screens for. I know the rate of false negatives is extremely low, but I guess I'm wondering if it would be worth the peace of mind to do something more invasive to have that certainty.

I felt really reassured by the negative NIPT. I was just turning 36 at my due date. I’ve posted this before, but I found this calculator helpful:

https://www.perinatalquality.org/Vendors/NSGC/NIPT/

I also felt like in most cases there would be something seen on my 12 week ultrasound (although I know it’s not always the case). Before NIPT I would have opted for CVS. I would have terminated for a chromosomal abnormality.

Op I could have written your post as I will also be 37 at delivery. I also had negative NIPT results but went ahead with amnio (totally easy and fine-not sure why so many fear monger about them) with a highly experienced provider and also did expanded microarray testing. Everything came back normal and now I feel like I can actually enjoy my pregnancy. It has given me peace of mind.

And for the record, I work with kids with various disorders and NIPT tests for only the most common things. Most of the kids I see don't have one of those things. That was really what pushed me to get the extra testing.

37 at delivery with my first, will be 41 at delivery with my second. In both cases, once I got the NIPT results back, I never considered amnio or CVS.

I was 35. Between the negative NIPT and the ultrasound markers, my provider said my risk was 1 in 10000 for a chromosomal issue. So yeah, it’s a screening, but between both, your chances of a false nahative are crazy low. I wouldn’t risk the, what is it? 1 in 500? Chance of a miscarriage against those odds.

The fact that it’s a screener means a ton of false positives, not a ton of false negatives. You might feel better if you get the exact numbers from your doc after the ultrasound is complete.

Why would you do the screening if you don’t the results?
The point is screen and then diagnose if screening is abnormal.

Yes, that’s the whole point. Talk to someone about your anxiety. No guarantees in life OP. They can pick up stuff at ultrasounds they can’t pick up through amino. Healthy fetuses can suffer oxygen deprivation at birth and end up with brain damage or cerebral palsy. If your NIPT was good, just relax. You’re not that old, and the risks are not that much higher at 37 than say, 30. If you were 47, maybe, but you’re not.

OP here, this isn’t about anxiety, it’s about the benefit of a diagnostic test versus a screening. I was looking for other moms who have considered this issue and wanted to get their thoughts. Obviously there are no guarantees with anything involving pregnancy and childbirth, or for that matter child rearing, but as a PP mentioned, there is an appeal to the level of comfort a diagnostic test provides as opposed to a screening. It sounds like several other PPs had similar concerns, so I’m hardly alone in thinking about these issues.

Seems like anxiety to me. I’d be surprised if insurance would cover an amino without a reason. That’s why they do the screenings first.

Most insurance companies have no problem covering amnio or cvs for women over 35. It's a normal test when you are AMA. As long as your OB orders it and states you are AMA it should be no issue.Because if there is in fact something that very wrong it saves them money in the long run.

Honestly I will never understand the reactions people give when moms want diagnostic testing. Why are you all so against it? The risk to the fetus is literally almost nothing with experienced MFMs. Get over it.

Would you terminate? That usually is a deciding factor. If you would change course and terminate then it makes sense to have definite information.
If you would not, it serves less of a purpose. No judgement if you would.
