3 mmc, the late one after IUI, now BFP without planning, what dr should I see?

Hi everyone, I am a bit lost, to say something. We have being trying to conceive for two years. This last year I started going to GW they did all the test nothing was wrong and we started IUI. BFP in the first try but ended in another mmc. I had decided I did not want to focus on that anymore and was trying to see the benefits of only one child... and now I just got a BFP. So scared, so lost on what do do. Should I go to GE? Should I go to my regular dr ( which btw did not like anymore given how they managed my mmc) can I go to Gw if this was a natural BFP? Any ideas would be much appreciated... I am really in shock and scared and of course so afraid to have hope and yet again Be crushed at 7 weeks. Thanks

You can go to a GW OBGYN or any other OB GYN. An RE doesn't monitor pregnancies unless it's very early.

Hi OP. Congrats on your BFP, and I'm really sorry about your mmcs. There are a few different directions you can go. It depends on what you're looking for over the next few weeks. If having early monitoring (serial betas & multiple ultrasounds) will give you peace of mind--the same process as you would have presumably had at GW IVF after the BFP from your IUI there--then for sure call them tomorrow morning and ask if they will monitor your unexpected natural BFP. My guess is that they likely will...I conceived naturally 2x while a patient there and they followed me (this was right after we completed our fertility testing; unfortunately one was chemical pregnancy, other was mmc, and I actually switched to a different fertility clinic afterward due, in part, to the way my IVF nurse and RE there handled my mmc. I couldn't handle going back.)

GW IVF essentially operates separately from the regular OB/GYN department at GW. You could go to the regular OB/GYN department there as a New OB patient. It is a very big practice. If you go this route, explain to them when you call that you've had 3 mmc, as that might trigger them to check your betas and do early ultrasounds...or it might not. To my understanding, their normal route for new OB patients is to just book you in for a confirmation ultrasound and first OB appointment at 8 weeks. After having 3 losses between 4-7 weeks myself, there is no way I would be ok being told to wait until week 8 for my first ultrasound.

Otherwise, you could (1) go back to your original OB, which sounds like a really bad idea since you've had a bad experience there with your previous mmc, or (2) go to any other regular OB office. I would guess that you may be more likely to get more early monitoring given your history of mmc at a small OB practice compared to a big one (i.e., the OB/GYN Department at GW).

FWIW, I'm 27 weeks pregnant now, and my OB is in the GW OB/GYN Bethesda office. I started with her there at 10 weeks after an IUI at the fertility clinic I switched to after I left GW IVF. If we were to find ourselves with an unexpected natural BFP in the future, I will, without a doubt, be calling my fertility clinic to ask them for early monitoring.

First, sorry for your past losses and fingers crossed for this one.

I don't think any regular OB practice will see you/follow you unless you are past 8 weeks, so if you're looking for any care now I would call your RE to get early U/S and BW done.

Do you have any understanding of what caused your previous mmc? If so, that might cause them to decide to do early pregnancy support such as progesterone suppositories or baby aspirin...most regular OBs don't know anything about these treatments, so you would need an RE to do them.

And PP is right that GW RE is totally different from GW OB. I finally got pregnant with my first at GW RE after 5 rounds of IVF, and they monitored me until about 8w. Then I switched to the regular OB practice there which treated me as if I were a brand-new patient (I think I even had to go through the whole rigamarole of transferring my records).

FWIW, if it were me I would call the GW RE clinic. It will give you peace of mind, I think.

Thank you all so much for your guidance. I will see if GW can monitor me. There are not know causes for my losses, everything seems to be normal. And generally I do not bleed and need to get a D&C...so you can imagine that right now I am looking at any possible sign that I am having a mmc again...hopefully I will be able to have some peace of mind soon...Thanks everyone!

Foxhall did monitor me with an early pregnancy (3-6 weeks) that ended in my. They did betas and an early u/s.

For current pregnancy (17 weeks, knock on wood), I had seen an RE (but conceived spontaneously). The RE did betas and more early u/s.
