Are you required to take a doctor note when you use a sick day?

How do you ask the doctor for a note? ...
Anonymous
No, but I do need notes for daycare sometimes, and I just ask the doctor to write it...

I work for an oral surgeon and people sometimes ask for notes. We have a form letter that we fill in. Or sometimes we just give the blank form to the patient to fill in themselves if they have lots of requirements for what they want written.

Your DC daycare? or the daycare located at 1600 Pennsylvania ave.?

Nope. Not unless I'm using FMLA.

No, but my doctor's office always offers to give me one for my office. The pediatrician's office offers too.

Some of our staff are. We have a form that must be completed and it has to certify that an examination was performed.

Nope. I'm an adult who works for and with adults

If your boss is asking for a note, they can do this, but they are being an anal.

Just simply tell the doctor that your employer requires a note. It's not unheard of b/c many times people do abuse sick leave. Just shrug and say, I don't get it either if the doctor is confused.

What if you are out sick with a stomach bug? I would never go to the doctor for that! For the most part because it's too risky being too far from the loo at any point in time!

Nope. Never in 22 years of working in law firms.

Doctors are used to having to provide these. Just say "I need a note that I was here for work," and they'll whip one out.

I think you are supposed to after 3 days but I have never been asked.

Technically we could be asked for one, but I have never been asked, nor have I asked anyone else. The only time I ever gave a note was when I had a high-risk pregnancy and had to either work from home or go on extended leave.

Only if we use two or more in a row. I just asked my doctor for something that said I couldn’t drive and had to be off my feet when I had a foot surgery and nobody gave me a hard time, even though it was a situation where I probably could’ve done desk work. I carry a lot of files back and forth and drive to other offices frequently so it was just easier to take the time off, which I’d saved up.

Only if I use more than 3 day in a row, according to the rules.
