Do PGS or not?

Age 33, history of ectopic pg that looked great beta wise and several chemicals. Have hence forth discovered MTHFR and multiple blood clotting issues.
First IVF and planning on doing Prednisone, Intralipids, Lovenox and PIO and Metanx (a cocktail to treat the discovered issues).

Do I really need PGS on top of this? Insurance doesn't cover and would count the need to freeze and then do a FET after testing as 2nd attempt out of the 3 IVF they allow per live birth.

My idea is to do a fresh IVF, we are doing ICSI, see how many make it to blast and do a fresh transfer this time of hopefully 2 good looking embryos and see what happens.
Is this stupid?

No, at your age I would have done the same. PGS is usually done if your first transfer doesn't work, unless there are other concerns like severe male factor issues. Wish you good luck!

I'm your age and currently in my third trimester. I did pgs and I'm glad I did. After going through so much for IVF I felt like it made sense to go all out and elect for pgs.

I had roughly 70 percent of the embryos (over 35) come back as normal. Had I not done pgs then this means there is a 30 percent chance an abnormal embryo would have been transferred and I most likely would not have ended up pregnant.

I know it's expensive but so is daycare. Pgs is a drop in the bucket in terms of what you'll spend on IVF and having a child. I'd do it so you don't waste time, effort and money transferring abnormal embryos.

Also if you pgs and the transfer doesn't work you know it's not the embryo. You know it didn't work for another reason and then you can try and find out why.

I'm doing IVF because I had two abnormal pregnancies. If I could spare anyone the agony of getting to week 12, then finding out about significanf chromosomal problems, I would.

Chromosomal problems are responsible for most miscarriages. I would absolutely recommend PGS. You're spending a ton of money already; why waste it with an embryo that doesn't have a chance?

+1 I wasted two fresh transfers with untested embryos because my RE said I'm young I don't need it. Switched to a new RE and did PGS and it did work after 2 chemicals.

In your situation I wouldn't bother. You can always PGS test any frozen embryos later. PGS is not a silver bullet - I had two chemical pregnancies with PGS normal embryos. Currently pregnant with an untested fresh one.

Of course not. There is no such thing.

But now you can isolate your particular issue, rather than abnormal embryos being in the mix.

Do you know why you had chemical pregnancies with PGS tested embryos? I had that as well. Just had 2 in current round of trying, and had 1 before I had my DD, also with a PGS tested.

PP with the two chemicals from PGS normals. I only produced a couple of blasts that round, and they were both normal, so for me, it wasn't like PGS saved me from transferring abnormal embryos. They never isolated my particular issue. I don't know why the untested fresh worked and the two frozen PGS normals didn't, and neither did the three different doctors I spoke to. When I asked, the response was a bit like 'you're pregnant now, what else do you need to know?'

Thank you for this article. It makes it so hard to decide. I am on IVF cycle #6 and finally decided to do a freeze all and PGS, but I've been thinking about all these factors from the article and also another study I read that says in some cases embryos can self-correct a genetic glitch as well as that the inner mass does not necessarily have the same defects as the outer shell (from which the biopsy is taken). Plus, PGS does not detect mosaicism, so there is that to consider too... So difficult to decide.

I had a pregnancy with a chromosomal abnormality so when we did IVF, we PGS tested the eggs. Out of 13 frozen, only five were normal. Like others have said, you're spending a lot of money so why chance it?

because 1) PGS is not 100% accurate 2) could hurt the embryo. real doctors (e.g. see article above) have doubts about using it for women with few eggs.

This is an interesting thread for me as I noticed that PGS testing is getting to be a norm now. Two years ago at age 35, I did a fresh cycle, and my doctor did not even bring up PGS testing and I didn't even know what it was. I conceived my daughter successfully, and have 6 frozen embryos. This time around for FET, I asked him if I should do it and he acknowledged that almost everyone is doing it, but he didn't see a reason for me to do it. I didn't, but I always wonder if I should.
