Some strokes are completely invisible from the outside, OP, so he could very well have had multiple ones without anyone knowing. This is how my grandfather developed dementia - because of high blood pressure which led to undetected mini-strokes.

I know I develop a sort of rictus if I smile too much, and then a corner of my mouth starts to tremble, because my muscles aren't used to it. I hate being under the public eye for long.
