That photo was taken at mar-a-lago around the time he'd begun dating her. It was presented to the book by a long time club member. They are mocking trump and that seems to be the explanation. Obviously there were many things going on in Epstein's orbit, but a tall woman who can't even pass for underage is fully deprecated in his ledger.
