What should I/we make sure to do before child leaves for college?

DD was on BC long before she was sexually active or anywhere close to going off to college. Since we didn't drink we didn't have to worry about that and two years into college we still don't. She did just break up with her live-in BF and she handled it very well. We skype every week and it helps to keep a line of communication open.

I read somewhere that I need a form to have access to DD medical information and be able to make medical decisions if something happens. Is this an official form?Where do I get this?
