This just happened because a sitting Congresswoman from Indiana who was running for re-election just died in a car accident the other day. I think the state republican party chooses a replacement to go on the ballot. If it is late enough before the election that the ballots have already printed, the deceased’s name remains on the ballot. Rather famously, John Ashcroft lost a Senate seat in Missouri to Mel Carnahan who had been killed in a plane crash only a week or so before the election, then W. made him his attorney general. Carnahan’s wife ended up in the Senate seat. Senator Paul Wellstone of Minnesota also died right before his upcoming election, but IIRC he lost.
