Fresh IVF cycle with PGD?

Anonymous
I had never considered PGD testing until I miscarried after my last IVF round. I'm 39 so I started looking into it since my egg quality is most likely part of the problem. I have 2 frozen embryos, which I've been told is not advisable to thaw, biopsy, and refreeze so I can't test those. I'm only covered by my insurance for IVF for 3 more months, so I am trying to squeeze in one last try with a fresh round and pay out of pocket for the PGD testing. However, my Shady Grove Dr told me they only do frozen transfers with PGD so I have to stop the meds after retrieval, freeze the embryos, and then go through another round of drugs for a frozen transfer a month later to give them time to get the test results back. Do other clinics do it this way or will they let you do a Day 6 transfer without freezing the embryos? Just wondering what other practices do, and any advice on whether its worth the expense for PGD testing on one round. I hope I can make that decision at the last minute after we know how many embryos we have to test.

CFA said they would thaw and test my 6 embies--which I had from SG but left for a variety of reasons. I wonder if you should do another fresh cycle, egg bank, test all embies, and then do an FET. I say that because you get the IVF paid for--that is the most expensive part!--and the FET should only be 4,500 (at SG) plus meds (meds are so much easier for FET!).

PP--I just realized that you may be able to test and transfer with Fresh as well, and if you have all of them tested, you will have a shot at an FET if the fresh cycle doesn't work. Hope that made sense! I hope it works!

Shady Grove and CFA both use Natera to perform the analysis on the 2-3 cells that are biopsied. They are located in CA, so it would be a really tight timeline to get the samples out there and run them through testing.

FET does extend the length of the entire IVF cycle a grueling amount and you have to manage your expectations. I am in my early 30s and endured a 3 months PGS cycle that did not result in a baby. Hope you have better luck.

I'm with SGF and they do a day 5 biopsy and fresh transfer day 6. Not sure why they told you you couldn't??

Pp here. I used IVIGen for embryo testing (pgs) and they are in FL. Maybe that's different than pgd testing

Shady Grove sent my embryos to Ivigen for PGS last month. They will give you the results in time to do the day 6 transfer, if other conditions are right. So you can complete everything in the same cycle.
In my case the progesterone rose before retrieval, so they were going to freeze anyway.

Some clinics do PGD testing using day-3 cells and transfer on day-5. I don't know which local clinics do that.
Doing PGD is a good idea, but recently I have heard so many stories where miscarriage happened even after transferring tested embryos. Results from m/c testing showed normal fetus. PGD has nothing to do with egg quality. It only makes sure the chromosome matching is normal.

SG definitely does day 6 fresh transfers for PGS (I'm guessing they could do the same for PGD) . I had my embryos biopsied last month on day 5. They were sent down to IVIGEN in Miami and results were in the next morning.

I did PGD on a 5-day embryo at GW. I don't believe any clinics in this area can do a fresh transfer if you're doing the testing on a 5-day embryo. They have to send it out for testing and don't get the results in time. Or at least this was the case in 2013.

So I did what you described...fresh cycle and then frozen cycle for transfer. This worked for me at age 38 after having 2 chemical pregnancies and several BFNs.

The testing on the 5-day is much more accurate than testing on the 3-day. The success rates are pretty high for 5-day normals. If you have enough 5-days to do this, I would.

Good luck.

OP here. My SG Dr said he didn't do day 6 transfers for PGD so that's interesting that others have been allowed. Does each Dr in the practice set their own standards or policies? My nurse said it was possible "but he didn't like to do it" and he told me no when I asked him yesterday. they send the embryos to Miami for testing so it seems they could get the results in a day and transfer on day 6. I love my Dr so I don't want to get on his bad side by pressing him too much bec he's been great through two prior IVF rounds but maybe I'll ask my nurse?

I thought the practice had standard policies. My doctor always talked about doing a day 6 transfer after getting the results from Ivigen.
