How do you ask for a promotion/raise during your mid-year review?

How exactly do you phrase it/at what point in the conversation?

I had to do this several times (in order to get one much deserved promotion).
I waited until the end - partly to gauge how the conversation was going to go, and partly because my company has a few standard questions/forms and one part asks about "anything your supervisor should know" or something like that.
I came prepared with "Right now I am X, I have looked at the rquirements for Y and here's how I think I meet them. Do you agree? Is there anything that you think I need to work on first (like develop ABC skill) or can we start this process?

This is good advice. In my company we have a document that spells out exactly what is expected at each level. I've had several people ask during reviews what they needed to do in order to get a promotion. I was able to use this document to chart out where they were deficient and put together a plan to rectify the deficiencies. Check to see if your company has something similar.
