This really does depend on jurisdiction, especially when it involved parking.

It does, but generally, the person parking or pulling out of a space needs to watch where they are going, so the responsibility is on that particular driver.

Car B attempted an unlawful pass. Doesn't matter what Car A was doing in terms of backing up or going forward. Not only is Car B at fault they would be lucky not to be ticketed.

If car A is backing up and hits the front of Car B... car A is at fault

If car A is backing into the spot, its rear is in the spot and Car B hits A's side, Car B

If car A is backing into the spot, its rear is in the spot and A pulls out because they misjudged the turn, and B tries to pass A... car B is at fault.

A person pulling out does not have the right of way. A person pulling in from a lane does have the right of way in that lane. So the person who attempted to pass another car that was currently occupying the lane would be at fault, because they did not have the right of way. Yes, someone parking needs to watch where they are going, but that's not the same as legal fault or responsibility.

How many lanes was the road? If it was a single lane, Car B made an illegal pass. If there were two lanes, why didn’t car B go fully into the second lane?

I had this happen, Car B was found at fault. I was at a stop and going to re-adjust, but car B couldn't wait, thought they could get around me, and were the ones to cause the swipe.

In DC cars are required to yield to cars that are parallel parking so if the signal was on then this is 100% on car B.

This is a toughie.

I personally get annoyed when the car in front of me has stopped immediately to parallel park.
I always thank my lucky stars I was very alert.

But my annoyance could stem from the fact that I have never mastered this very important skill.

I would say fifty/fifty??
