Madewell slim cut flannel shirt
Anne Taylor always skinny jeans
scarf i bought in Madagascar (similar colors to the shirt)
LL bean purchased black boots (they were called low boots when I bought them last year but now upgraded to "booties" apparently)

Ann Taylor Loft and The Gap both sell this style of pants. Flat front, high waisted. I think BR & Jcrew do as well.
