Don't forget places like India and Philippines. I have friends who retired in both places. English is widely spoken and costs are low. One of my ex-coworkers moved back to India and he's now widowed, and has a staff of 5 at his house, including a driver, gardener, cook, and maid.

My mom has exactly this. She in her 70s and lives in Los Angeles, and it’s very common in her age and wealth bracket.

I have family members who travel to Thailand and Turkey for medical care and are very happy.They are destination countries for medical tourism.Agree Trauma care in hospital, cancer treatment and some specific surgical procedures US might be better if you are well insured.

Have a meeting, explain that it's time for her to learn to take care of herself, agree what changes each of you will be responsible for, and start the next day. Either or both of you can suggest changes and both must agree to the changes (or not). You'll be doing her a favor too, but she may not see it for awhile.

My grandma has someone like this to drive her everywhere, do grocery runs and prep food, tidy up, make sure she remembers her meds, that sort of thing, for 4 hours a day. She is pretty independent so they don’t do much caretaking.

Yes, we just hired a companion for my MIL. She comes five days a week. Does a few light chores, but mostly her job is to drive and to keep MIL mentally stimulated.

In addition to pay, she gets two meals a day, gas and mileage, and MIL pays if they do things that have a cost.

When I visit my 2 older aunts in Latin America they all have this type of person (each lives in their own house) and so do several of their friends.

It is their maid they hired years and years ago. They all have a younger maid who does all the hard work and the older maid hangs out with the aunts. Makes breakfast for them then they chat over coffee, goes with them to shop, encourages them to finish tasks, advises them which outfit is best, gossips about neighbors, cleans their room, does their laundry, etc.

You described my aunt’s nursing aide. She is wonderful. She cooks her meals, does her laundry, cleans, runs her errands and is wonderful at keeping her entertained. She even changes diapers.
