I’m a pretty minimalist/declutterer, but I think that if you have decluttered to the point where this is the most on the bubble piece of paper clutter in your house you may have gone too far.
I have a single file drawer of important paperwork and it contains a section of medical records and my vaccine card and that of my husband and kids lives there. |

That's trickier in this area where you could easily get a shot in DC at the CVS or doctors by your office and another one at the CVS by your house in the burbs in another state. I have doctors in three states only 5 miles apart. My kids have each received shots in two different countries, which caused untold problems for these computerized systems. |
