Giselle is no fool. He is at major risk of all sorts of things from his concussions-mental illness, early dementia, etc. She was willing to take those risk as part of the deal for only so long. Now with another season, he increases risk of disability even more.
As a parent of a child with medical issues and a disability and the daughter of a volatile mother with mental illness, I have also been through dementia with a parent and none of this would be a walk in the park with even all the money in the world to throw at it (though it would be easier). If she stays married and this season gives him another head injury that tips the scales she could within the next 10-15 years be looking at a life of hell. The press will eviscerate her for not doing enough for him as he declines. She will be eviscerated for going out and having fun with friends while he is home disabled even if it's self care. She will have a partner who not only doesn't do his part, but drains every ounce of her energy of he becomes moody and abusive or declines into depression or develops dementia. If she divorces him once he becomes disabled, she is a monster. If she divorces him and he declines into disability thanks to football injuries, she is a saint in the press if she arranges care and checks on his now and then and stays in his life. She can find a new husband and she is still considered a good person because she doesn't abandon him. |

I'll add that for OP's question it doesn't matter. Many people in intense careers make it very clear to their spouse that their careers come before the marriage and kids. Or at least they make clear that they are never going to stop doing what they are doing. This isn't the case with Tom and Gisele. |
