I saw that same interview last year (Jane Pauley on the back of his motorcycle!) and his chain-smoking was my first thought, too. Meg Ryan smokes as well, but it can't be as much as he does (four packs a day at one point).

I thought I'd read somewhere that he'd had a multiple-pack-a-day habit, but had had some sort of episode (heart attack, I think?) numerous years ago and quit?

He had a heart attack in 1994 and cut back a bit.

Her plastic surgery semis to have calmed down but I couldn’t help but think she looks vaguely like Christie Brinkley - who also dated Mellancamp. Wish I was just a little older because I would have hunted him down myself. Viva Meg!
