Lupron has totally opposite effects depending on how it's used. In a long lupron protocol it suppresses follicle development. In Lupron flare, it increases FSH levels to stimulate follicle development. The Lupron stop or micro-dose protocols are also suppressive, but less so than long lupron. You wouldn't generally use long lupron for DOR/poor response. Estrogen priming is another good DOR protocol that is often used. Whether one works better than the other can often depend on the person. I did well with estrogen priming (lots of eggs, but not good quality, so no success), but lupron stop was dismal. Cornell also uses a min-IVF-like protocol and says they typically get close to the same # of eggs as a more standard DOR protocol. I think the idea is that the very high doses of stims aren't good for egg quality, so perhaps its more relevant for DOR with AMA. If you're young egg quality tends to be better even if you have DOR.

I was told by an RE that the protocol used does not affect egg 'quality' -- just the number retrieved and thus indirectly affecting chances of success.

Just came across this thread! I'm about to start my first IVF cycle at CCRM Nova with Dr. Payson.

32 y/o, TTC for 2 years, DOR (amh .9 and FSH 12), AFC showed 8 antral follicles, elevated prolactin (50~100), and some autoimmune thyroid issues but currently normal levels of TSH and T4.

Supplements: prenatal vitamin, fish oil, CoQ10, DHEA, and Vit D

I start priming with birth control pills this week... fingers crossed! Anyone else in the same boat?

I’m a semi-new patient seeing Dr. Moragianni for my 4th IVF attempt. So far, I absolutely love her. She does a great job at balancing hope with realism, all while maintaining a sense of humor. I previously cycled at GW 3 times. I did 7 transfers there – 4 negatives, 3 miscarriages. I had my first egg retrieval at CCRM last week, and out of 13 eggs retrieved, 9 made it to blast for CCS testing. This is by far the best result I’ve had thus far in my journey. I can’t say whether it was the lab, protocol, or perhaps just plain luck this time, but I will say the overall process at CCRM has been as smooth as can be, largely in part due to their very open and clear communication channels. I feel like a priority patient, even though I’m new, and it feels awesome. Good luck, everyone! I really think we're all in great hands!

Did you do PGS/CCS testing at GW? How old are you? Which protocol are you using at CCRM? Sounds like things are going well for you at CCRM - best wishes!

I did not do PGS at GW. Of my 3 miscarriages, we know one was normal and one had Turner Syndrome from D&C findings. I’m currently 29 (but was 27/28 when I cycled at GW) and thought I wouldn’t need it at the time. Hindsight is always 20/20.

I respond pretty weirdly to medication. My cycles at GW were all over the place: 3 eggs with no blasts, 20 eggs with 6 blasts, and a cancellation. At CCRM, I primed with estrogen and was on the highest dosage of medications I’ve ever been on + Clomid. Dr. Payson will closely review your history and thoughtfully come up with an individualized plan best suited for you.

I hope you’ll be as happy with CCRM as I am. The lab, facilities, and staff are topnotch. Good luck to you, too – you can do it!

How do they do monitoring at CCRM? Is it like GW Where you have to arrive between 7-7:45 am?

You have to make an appointment so you get a set time. Monitoring is between 7:30-9AM though they once let me come in later at around 1PM because of a scheduling conflict which was nice of them.

I also want to add from my experience, my doctor did every one of my monitoring appointments. They didn’t rotate like they do at GW. They’d let me come in around 10:30 to avoid traffic, as I’m was coming from Baltimore.

Another perk - I was able to do my first week of monitoring in Columbia (sono at a Progressive Radiology and bloodwork at LabCorp). It was pretty easy, and I really appreciated that it saved me some driving time.

What I hated most about GW was waiting in line like cattle and that their OB appointments occurred during regular monitoring hours. I got pregnant 3 times there, but miscarried each time. Being told there wasn’t a heartbeat to then go stand in a line for blood work behind other women was absolute torture.

^Same pretty much. I'm the one that commented about monitoring hours and Dr. Payson did almost all of my ultrasounds/procedures over 3 cycles. I did get Dr. M on a weekend a few times but that was fine. I so appreciated having my own doctor do that stuff and have time to chat with him if I had concerns. Also a nice place to sit in their lobby. I felt like everyone knew me and was in this with me so definitely think you're in good hands there.

Just had a retrieval and same number of eggs at CCRM. That is awesome that 9 made it to testing! Do you mind sharing when you know how many are normal? Good luck to you!!!

I got my results on Friday, so about 1.5 weeks since the biopsies were sent. 6/9 normal. I’m more confident than ever in CCRM’s lab based on my recurrent miscarriage history. Good luck to you!
