My guess would be the comments come from peers or the images that society promotes these days. I was shocked at how young my sons and many of their friends were when they started caring about six-packs and looking hot. It's a sad commentary on our society, but unfortunately, all the social media (and the pictures they post - bikini pics and topless pics of boys are not uncommon in 6th grade}. Unfortunately, I think we're going to see a lot more boys developing body image issues. I agree with PP, tell your pediatrician and have him/her talk toyour son.

We are monitoring my 13 DD for eating disorder. She is very thin and focuses too much on what she consumes given how extremely active she is. The pediatrician has been very helpful. I recommend going there first.
