If you work from home and have young kids, how do you handle childcare?

I think a nanny share at a neighbor's house would be perfect for you! Or a small in home daycare.

Daycare. It is nice to have the option to have baby out of the house so I can focus on work without distraction. It is hard to not be somewhat in tune with your child when they are in the house. And on days off, I can still have a day sitter. DD loves the center she is at, doing really well, and I love the teachers.

I would work with a nanny. It's what your child will know from birth so they won't know any different. It will make the nursing SO MUCH EASIER. Just set up firm rules, that your nanny can only knock on your office door if it is a true hospital type emergency, etc.
In the beginning the baby is sleeping so much it won't know that you are in the house unless you are in the same room. Once they become aware your nanny is going to get out of the house for classes, play dates, playground, etc. so it will be less invasive. And if they get to the age where they are aware you are in the next room and can't be disturbed you can start to look for a nanny share. I would consider the backyard shed deal after the second kid comes along or at least wait until after the first year.
Day care or nanny share at someone else's house would just be a pain to be if I truly wanted to breastfeed. I hate pumping.
