Hard one to navigate - I have a friend obsessed with her weight, who actually is overweight, and she eats pretty poorly while complaining that she works so hard yet gets no results. I like PP's approach of asking if she's alright and in need of help or if she's just looking for reassurance.
I usually don't know what to say to this friend. I could point out that a diet full of muffins and pasta is not the easiest weight loss path, but there really is no polite way to say that. On the other hand, she's really bummed about her weight, so I feel bad not telling her how she could make things easier on her self. She occasionally asks me how I lost the baby weight and I tell her but she concludes it must just be the genes. It's hard to be around her when she gets down so on herself but won't take the concrete steps needed to change her situation.
