Oh, and fully clear out any personal items from your desk/ cube / office BEFORE you give notice. At least get them inside your car first.

Many business will escort the employee out the door immediately once notice is given. (And also cut off employee from email and other IT access).

Agree. Give notice to your boss and then go talk with her boss to tell her what a pleasure it was to work with her, etc. and leave anything negative out about the direct boss.
