IMO (and in my experience), when parents and teachers are on a first-name basis, boundaries are blurred.

Keep it professional. I've corrected parents who addressed me by my first name.

I start out calling the teachers Mrs. X, but when they address me by my first name, I take that as a sign that it is ok for me to do the same.

Oops - sent too fast. Also when they sign emails or id themselves on the phone by their 1st names.

Always be Ms. or Mr. unless invited to use the first name. Also, until I invite teachers to call me by my first name, they don't.

"Mrs. X" because that is just how I think of her, since my kid says her name a million times a day!
