I screwed up and somehow this feels like the straw that broke the camel's back

Go to target or cvs and get it done. Especially if you encouraged your child to do this. You can have it today!

I agree, call and beg for mercy from both the pediatrician office and the school.

How serious can the cuts be for middle school sports? There’s gotta be a team he can walk on to late.

CVS minute clinic! DCUM saves the day.

This. Make an appointment online. Find the nearest one to you with an appointment today. I was in the same boat in September and the Minute Clinic was easy and fast.

send a note to the coach/head of athletics and tell them what's going on.

There's some good advice on this thread already about trying other doctors, contacting the school/coach, and emphasizing you're not a bad parent (you're not! And ignore the poster who preached about not believing about letting their kids suffer for their mistakes, that person is a dumbass and isn't sending their kids the message they think they are). And honestly, if you've got a very anxious kid you were talking into trying out in the first place, there's a good chance he'll be incredibly relieved to have this pressure off of him without it being his fault.

Does it need a stamp from the office or just an illegible signature? Otherwise things could happen...

The sports teams at the middle school my kids went to were very competitive; many kids were cut.
Yes, there are always “pay to play” rec teams, but OP is talking about the actual school team.

This - it’s something like $80 and they’re set up to do exactly this.

Patient First and some CVS Minute Clinics will do this for you.

I don’t believe in letting my kids suffer for my mistakes, so saying oops, you can tryout next season/year doesn’t fly for me. Suck it up and pay the Monday at RightTime (or wherever).

So nice. I don’t believe in me suffering for my mistakes but sh*t happens.

It's 50 dollars at Patient first.

Thank you all so much, for the lack of judgment and the very helpful advice. I never ever would have thought of Minute Clinic or urgent care, although now that you all have suggested it, it seems obvious. I really don't know why this hit me so hard, but everything has been a lot lately, and to blown something so easy (dropping off a form!) seemed like the ultimate failure.

I posted mainly just to get it off my chest but hopefully some future panicked parent will find this thread when they're trying to figure a way out of a similar mess. Thanks DCUM, you're a wonderful lifeline!

Funny how many of us have been in this boat. We have resorted to the last-minute urgent care clinic sports physical many a time!
